Accessing Your Care Notes at AnsMed Home Care
At AnsMed Home Care, we believe that transparency and communication are essential to quality care. That is why we provide clients and their authorized family members with real-time access to care notes, allowing you to stay informed about the care being provided every day.
This article explains what care notes are, why they matter, and how you can easily access them.

3. How to Access Your Care Notes
AnsMed provides a secure and user-friendly platform that allows you to access care notes from your mobile device or computer.
Step 1: Download the App
Visit the Google Play Store (for Android devices) or the Apple App Store (for iOS devices) and download the ContinuousCare Health App.
Step 2: Contact Your Care Manager
Inform your Care Manager or Care Coordinator that you would like access. They will activate your account on the system.
Step 3: Set Up Your Account
You will receive an email with instructions to create your password and complete your account setup.
Step 4: Log In and Access Your Notes
Once your account is set up, you can log in to view daily care notes, updates, and other relevant information.
Optional: Access via Web Browser
You can also access your care notes through a web browser using your login details, providing flexibility across devices. Use https://ansmedhomecare.continuouscare.io/ to access your care notes through the browser.
4. Important Information to Note
- The email address provided during the initial assessment will be used to create your account
- For security reasons, the email address cannot be changed after setup
- Access is limited to authorized users only to protect client confidentiality
